In consideration for being allowed to participate in the above-referenced Activity, and on behalf of myself and my successors, next of kin, heirs, representatives, and assigns, I hereby release, waive, hold harmless, and discharge from all liability and promise not to sue Stone Ridge Asset Management LLC or its employees, officers, subcontractors, volunteers, representatives, and agents (collectively, “Stone Ridge”), or the Conference or its organizers, employees, officers, subcontractors, volunteers, representatives, agents, and premises owners or lessors (together with Stone Ridge, the “Releasees”) from any and all liabilities or claims, including without limitation claims of negligence, based upon, related to, or arising from any injury, including without limitation any paralysis, disfigurement, pain and suffering, illness, damages, property loss, economic or emotional loss, death, or any other type of harm, that I may suffer because, as a result of, or in connection with my participation in the Activity, including travel to, from and during the Activity.

I am voluntarily participating in this Activity. I further acknowledge and accept that participating in the Activity carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid such risks. I am aware of the risks associated participating in this Activity, which include but are not limited to physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and/or death. I understand that these injuries or outcomes may arise from my own or other’s actions, inaction, or negligence, or the condition of the Activity or Conference location. Nonetheless, I assume all related risks, both known or unknown to me, of or related to my participation in the Activity.

I agree to indemnify and hold Stone Ridge harmless from any and all claims, actions, suits, costs, expenses, legal fees, and liabilities for any injuries to myself and for any damage to my property or possessions that arise out of or from my participation in the Activity. If Stone Ridge incurs any of these types of expenses in connection with my participation in the Activity, I agree to fully reimburse Stone Ridge. If I need medical treatment, I agree to be financially responsible for any costs incurred as a result of such treatment in connection with my participation in the Activity. I am aware and understand that I should carry my own health insurance accordingly.

I am 18 years or older. I understand the legal consequences of signing this document, including (a) releasing Stone Ridge from all liability, (b) promising not to sue Stone Ridge, (c) and assuming all risks of participating in the Activity.

I understand that this document is written to be as broad and inclusive as legally permitted by the State of California. I agree that if any portion is held invalid or unenforceable, I will continue to be bound by the remaining terms.

I have read this document in its entirety, fully understand its terms, and acknowledge that I am signing it freely and voluntarily. No other representations concerning the subject matter or legal effect of this Assumption of Risk Acknowledgement and Release and Waiver of Liability have been made to me, and I have not relied on any such representations.
